IMPORTANT PAGEANT INFORMATION

• The Competition will be held at KY Fair & Expo Center, North Wing - 937 Phillips Lane Louisville KY 40209.
• Contestants are responsible for admission & parking costs.
• Adults: $14 Ages 6-12: $5 Kids 5 & under: FREE! Parking: $6.
• Winners will be announced after each group competes.
• Little Miss & Mr., Pre-Teen, Teen and Miss Winners must provide birth certificates to the State Fair Board.
• No experience necessary. This event requires no speaking.
• Contestants are scored on appearance and presentation.
• Cash only at registration.
• Free Optional Photogenic for Miss Contestants ages 18-21 only. Two winners will receive a banner and a $1000 modeling contract with Creative
  Management Group. This is a B&W or Color photograph you bring with you to the event. If you are already with CMG, you may not enter the Photogenic
  portion.
• The Car Show is massive. Please allow yourself at least an hour to park and get to the stage!
• The dressing room is for contestants only. If you bring guests backstage, you will be disqualified.
• Entrant agrees that this entry form, once signed by entrant, supersedes any and all prior agreements and understandings, whether written or oral between
  entrant and the contest, the producers of the contest, namely Denise Mattingly, WDJX & Carl Casper’s Show Cars, Inc.
• As between the Producers and the entrant, the producers shall have the final authority to accept or reject the entrant, to determine at what time the entrant
  shall perform, the length of the performance, the selection of the judges and any and all decisions concerning the Jefferson County Pageant.
• The entrant agrees to abide by the contest judges’ decisions and accept such decisions as the final word on any and all issues. Entrant further agrees to
  conduct his/her self in a very positive, discreet and polite manner.
• Unsportsmanlike conduct by the entrant or entrant’s supporters will result in disqualification from all Denise Mattingly events.
• The entrant agrees to waive, indemnify and hold harmless the Producers and all other associated parties to the contest for and against all demands, claims or
  lawsuits that may arise from the entrant’s participation in this contest.
• All winners shall be personally responsible for all state and federal taxes on their prizes received.
• In consideration, Winner(s) give Carl Casper and its sponsors the right to use their name and likeness for advertising, promotional purposes and other
  materials produced. They agree to abide by all rules & regulations or all right to prizes, etc will be forfeited.
• If Winner does not compete at the State Fair Competition, all prizes & considerations will be forfeited and the Winner will be disqualified from all Denise Mattingly pageants, and a service charge may also apply.
